CCOA SERVICES
Home Delivered Meals
A hot nutritious meal is delivered 5 days a week, Monday through Friday to persons aged 60+ who are homebound by reason of illness, incapacitating disability or are otherwise isolated. Suggested donations for this service is $1.30 per meal.
Congregate Meals
A hot nutritious meal is served at 11:00am until 12:15pm Monday through Friday at our Senior Center on 307 Main Street. TRIO Community Meals caters all our meals. We ask that you call us a day in advance so we can order your meal. Suggested donation for persons 60+ is $1.30 per meal.
Homemaker
One hour of light housekeeping task in the home of persons aged 60+. Light housework: dusting, wiping surfaces, cleaning spills, sweeping, mopping and washing & drying small loads of laundry. Suggested donation is $1.00 per visit.
Transportation
Our transportation service operates between the hours of 6:00am to 4:00pm, Monday through Friday. We provide curb-to-curb transportation services to doctor appointments, grocery store, work sites and more. All of our vehicles are handicapped accessible.
To schedule transportation services please call 318.649.0107 at least 24 hours in advance. The suggested donation for persons aged 60+ is $1.00 per trip. Riders under the age of 60 are required to pay a cash fare as per the fare schedule. Fare Schedule Here
Recreation
We offer a variety of recreational activities at our Senior Center including dominoes, bingo and much more. Activities usually begin at 10:00am unless noted otherwise. All Activities are listed on our monthly Activity calendar.
Legal Services
For legal services, call Acadiana Legal Service Corporation at 866.275.2572, to answer any questions regarding Wills, Power of Attorney, Divorce, etc.
Other Services
CCOA offers other services such as Nutrition Education with a Registered Dietician, In-Home Respite, Health Promotion and Disease Prevention, Material Aid (Food Boxes), Information & Assistance, Outreach, Medical Alert referrals, and Energy Assistance through the Power to Care Fund.
Support Coordination Services
The Caldwell Council on Support Coordination Agency coordinates services based on an individual’s needs. These needs are met with a comprehensive plan of care to maintain a quality of life. Our objective is to provide case management services assuring the health and safety needs of recipients; coordinate services, generic community resources and other natural supports needed to assist the recipients to achieve or maintain their desired Personal Outcomes.
Support Coordination Contact Information
Each Medicaid recipient of Medicaid Waiver and targeted Support Coordination has the freedom to choose their providers. The individual’s choice of Support Coordination or direct service provider does not affect their eligibility for waiver and state-plan programs or support coordination services.
Medicaid recipients receiving support coordination services through the Office of Aging and Adult Services (OAAS) may contact the OAAS help line at 1-866-758-5035 for information, assistance or questions about OAAS programs.
