CCOA Case Management Agency

The Caldwell Council on Aging's Case Management Agency coordinates services based on an individual's needs.  These needs are met with a comprehensive plan of care to maintain a quality of life.  Our objective is to provide case management services assuring the health and safety needs of recipients; coordinate services, generic community resources and other natural supports needed to assist the recipients to achieve or maintain their desired Personal Outcomes.

 

Normal business hours are Monday through Friday, 8:00 a.m. to 4:30 p.m.

Caldwell Council on Aging Staff are available to their clients 24 hours a day, seven (7) days a week.

Each Medicaid recipient of targeted or waiver case management has the freedom to choose their providers.  The recipient's choice of case management or waiver service(s) providers does not affect their eligibility for the waiver or case management services.

 

Recipients of Medicaid Case Management Services may report any problems with their case management services to the Waiver Management Complaint Desk at 1-800-660-0488.
